Run a chart audit

Work an audit, record findings, and send them back as tasks.

A chart audit checks a patient's documentation against a checklist. You work the checklist beside the source documents, then send findings back as tasks or return the chart to the clinician.

Find your audits

  1. Open Quality Management in the sidebar, then the Chart Audits tab.

  2. The Chart Audit Queue lists every audit with patient, episode, audit type, status, reviewer, flags, and last update.

  3. Narrow the queue with the filters: status (Pending, In progress, Returned to clinician, Completed), audit type (ICD & OASIS, POC, Chart notes, ICD), reviewer, or the search box.

  4. Click a row to open the audit workspace.

Work the checklist

The workspace has three panels: the Audit Checklist on the left, the source document viewer in the middle, and Key Findings with related rules, tasks, and the audit timeline on the right.

  1. Expand a checklist item. Each one explains what was checked, cites its Sources and Regulatory Basis, and opens the matching document in the viewer.

  2. Verify the item against the source — referral PDFs, OASIS assessments, visit notes, the 485, orders, wounds, and medications are all available as viewer tabs.

  3. Check Mark as reviewed. The progress counter at the top of the checklist tracks how much is done.

  4. For a problem that needs someone's attention on its own, click Create task on that item.

Close the audit out

The workflow bar at the top moves the audit through Open → Reviewing → Returned to Clinician → Completed.

  • Create tasks from findings turns every item under Key Findings into its own task in the Tasks queue.

  • Click Returned to Clinician when the clinician needs to fix documentation — a task is created for them automatically.

  • Click Completed when the chart is clean or every finding has been handed off.

Note: Queue statuses and workspace statuses use different words — Pending Review in the queue is Open in the workspace, and In Progress is Reviewing.

If something goes wrong

  • An audit you expect is missing from the queue. Click Clear filters, then search by patient or MRN.

  • You cannot click an earlier workflow step. Past steps lock once the audit moves forward. If a completed audit needs rework, create a task from the finding instead.
