The review workflow

Does the quality check update in real time as answers change? Yes — it updates as you go.

A clinician accidentally rejected a change. Can it be resent? Yes. Go back to the exact item and resend it — re-enter the reason, and it repopulates on the clinician's phone. The item shows what was changed, that it was rejected, and the clinician's rejection reason.

Can QA make a minor correction — say, an M-code fix — without sending it to the clinician? No. Every OASIS item modification routes to the authoring clinician to review and approve; QA cannot bypass that. The only override is for a clinician who is no longer with the agency. Does the quality check review every visit note, or just OASIS? At launch it runs on OASIS assessments inside the Review OASIS task. Full patient-level chart audits covering all visit notes live in Quality Management — see Run a chart audit.

Where do I review the plan of care's interventions, goals, and locators? The POC tab inside the Review Plan of Care task: general info, ICD codes, calendar and frequency, interventions, goals, procedures, and locators. See Review a plan of care.

Are the OASIS hover tooltips visible to clinicians? No — item intent, time points, and coding instructions are for the back-office reviewer. Clinicians get a completeness score at the end of their visit showing which areas the recording did not cover.

Who should hold the Clinical Manager role? A clinical person (RN or PT) in the office managing field clinicians and patients. Users can hold multiple roles for combined access — see Set roles and permissions.

Face-to-face

The F2F visit has not happened yet. Does the chart sit open? No. Use hold-for-additional-review on the F2F and still send OASIS corrections to the clinician in the meantime. With no F2F document, the screen reads "no documents eligible" and the chart is flagged as missing the requirement — address it when the document arrives.

Who can record the F2F outside QA? Intake can indicate it during intake, and it can be tagged in the patient chart. Neither disrupts the QA workflow.

How does the system know a F2F document is signed? Only documents with a valid provider signature qualify as F2F-eligible. An unsigned document is not offered as an option in the Review Plan of Care task.

How do I find an original F2F from years ago? The chart's face-to-face section stays with the selected F2F document across episodes — one click opens it. The Documents view filters by this episode or all episodes, and F2F-eligible documents carry a badge.

What happens upstream

What if a clinician completes a start of care without a plan of care? They cannot — the SOC cannot be completed without a POC. Scribe prefills the visit note and plan of care from the transcript; the clinician reviews and signs, and a review task lands in the back office. That review is the double-check.

A discharge OASIS — will the system remind me about incidents from the episode? Incidents live in the chart's tracking logs; check them when completing the discharge assessment. Automatic surfacing of incidents into relevant assessments is on the roadmap.

Locking and submission

Where does the OASIS export to iQIES happen? In the OASIS dashboard. Export the locked assessments and upload the file to iQIES; a direct connection for automatic submission is on the roadmap. See Lock the OASIS.

Can CAHPS survey data go to our survey vendor? Yes — CAHPS survey data exports for delivery to an external survey vendor.

Quality programs

What is a QAPI event and where is it documented? A trackable safety or outcome event — a medication error, a fall with injury, an infection, a death on service. Document it in the chart's Tracking Logs: structured and searchable, replacing the three-ring binder.

How current are the star ratings in Quality Management? Refreshed as close to real time as possible, unlike the 6–18-month-old data in most quality dashboards — recent changes show up fast enough to act on.

Where can I see what changed and who changed it? Every task has a clock icon opening its audit log. An agency-wide audit lookup lives in Settings → Agency Profile.
