Before a new plan of care goes to the ordering provider for signature, a reviewer checks it. Two task types open from Tasks into the same workspace as QA reviews:
Plan of Care Review — the full check: referral order, visit documentation, OASIS, and the plan of care itself.
Review and Lock OASIS — a focused pass on the OASIS alone; other tabs are read-only context.
The review task is the working surface. The patient chart and OASIS dashboard help you track the record, but the approval action belongs in the review task.

Work the review
Open the task from Tasks, or click Claim next. A Plan of Care Review opens on Referral Order and walks left to right; a Review and Lock OASIS opens on OASIS.
Read each tab, fix what needs fixing on the editable ones, and click Mark [tab] as Reviewed. The workspace advances to the next unreviewed tab.
On the POC tab, open Quality Check for plan of care quality flags, and keep Docs open with the referral order beside you — mismatches between the referral and the plan are the most common catch.
Plot the visit calendar
The Calendar & Frequency section on the POC tab is where the ordered frequency becomes plotted visits on the episode calendar.
Check the frequency chips per discipline — for example SN
2wk2, 1wk5or PT2wk6— against what the referral and evaluations ordered. Read the notation as times-per-week for a number of weeks:2wk9is twice a week for nine weeks;1wk1, 2wk8is once the first week, then twice a week for eight.Drag a scheduled visit to reschedule it within its week, or use Change Visit Type on a plotted visit.

Visits satisfy the frequency within the Medicare week — exact days can shift unless a day-specific requirement is noted. Perfect dates are not the goal: confirm the frequency matches the order, the visit types match policy, and day-specific requirements are captured. Schedulers move visits within the week, and clinicians can request reschedules from the app.
The plotted calendar is the frequency of record: what you confirm here is what shows on clinician schedules and what the 485 documents. Mid-episode frequency changes arrive later on their own provider orders, plotted the same way during review — see Approve a provider order.
Preview the PDF
On a Plan of Care Review, click Preview PDF to open the POC PDF Preview panel. It renders the plan of care exactly as the provider will be asked to sign it. After a change, click Regenerate to rebuild it. Review from the PDF, not just the form view.
Put the plan of care on hold
When the chart is not ready to finalize — most often a face-to-face encounter that has not happened or arrived — park it instead of completing with a gap:
Click Hold For Additional Review in the header.
The task moves to On Hold in the Tasks list. First-week visits proceed while the chart waits; nothing goes to the provider.
When the missing piece arrives, reopen the task from Tasks, finish the remaining tabs, and complete normally.
Note: A hold is a bookmark, not a resolution. Note what you are waiting on in Comments so the next person — or you, next week — knows exactly what releases it.
Complete
When every reviewable tab shows its green check, Complete Review unlocks. If you changed OASIS answers, the button reads Send Changes for Approval first — see Send changes to a clinician.
Click Complete Review. For a Plan of Care Review this finalizes the order and adds your Clinical Manager signature as a cosign.
Orders with a known provider go out for signature automatically and appear on the Orders page for Medical Records to track through signature and filing.
If something goes wrong
Complete Review is grayed out. Hover the button to read what is blocking. If it mentions the M0030 start of care date, resolve that finding in the banner above the tabs first.
A tab that should be editable is locked. Marked-reviewed tabs lock; use the undo button in the footer to reopen. If changes are with the clinician, editing stays locked until they decide.
The review completed but the order did not send. Automatic sending needs a deliverable provider. Open the order on the Orders page; if it sits under To Be Sent, send it from there.
The F2F has not happened yet. Use Hold For Additional Review to park the chart until the face-to-face documentation arrives, rather than completing with a gap.
