OASIS reviews open from Tasks into a tabbed workspace. Review and Lock OASIS is the working surface for the OASIS review. The OASIS dashboard is where locked and unlocked assessments are tracked and where export or iQIES follow-up happens after review.
A full review workspace can include Patient Info, OASIS, Diagnoses, POC, Face to Face, and Attachments. Related tasks (Plan of Care Review, Review and Lock OASIS) use the same workspace with fewer editable tabs.
Open a review
Open Tasks in the sidebar. Active lists open work.
Open a QA review assigned to you, or click Claim next in the header to pull the next waiting review. If you already have one in progress, it opens that one.
Work each tab
Read the tab. OASIS, Diagnoses, and POC are editable; Face to Face shows the source document to check.
When you change an OASIS answer, enter the required Clinical manager note, then click Save change.
When a tab is right, click Mark [tab] as Reviewed in the footer. The workspace advances to the next unreviewed tab. The undo button next to the reviewed badge reopens a tab.
Side panels open from the buttons right of the tab row:
Button | Holds |
Docs | The patient's source documents |
Notes | Your review notes |
Comments | Conversation on the review, with mentions |
Quality Check | Quality flags for the POC (on the POC tab) |
PDGM | Payment grouping preview |
Note: Open Docs before starting the OASIS tab so the source sits beside the answers you are checking.
Write the clinician note
The clinician note is the rationale the clinician reads when they accept or reject the change. Keep it specific and tied to documentation.
Good notes explain:
Why the current response is not supported.
Which documentation supports the recommended response.
What the clinician should learn or check next time.
Avoid short notes that only say "wrong answer" or "per QA." The note should stand on its own.
Finish
No OASIS changes: once every reviewable tab is marked reviewed, click Complete Review.
You changed OASIS answers: the button reads Send Changes for Approval. The changes go to the clinician's phone to accept or reject each one — see Send changes to a clinician. A Review and Lock OASIS task cannot finish until every clinician decision is resolved.
For Review and Lock OASIS, complete the final pass only when there are no outstanding clinician approvals, no denied items still needing action, and validation issues are cleared. Then mark the OASIS reviewed and click Complete Review.
Need more from the clinician first? Click Request Additional Documentation (on some review types, Hold For Additional Review) to put the review on hold.
If something goes wrong
Complete Review is disabled. Every reviewable tab needs its green check — the tooltip reads "Mark every reviewable section as reviewed first." Also look for an unresolved finding, such as a start-of-care date mismatch banner.
The OASIS tab is locked. You sent changes and the clinician has not decided on all of them. The banner at the top tracks progress; editing unlocks when every change is accepted or rejected.
Claim next says no work is available. No unassigned reviews are waiting. Reviews for recorded visits are created automatically as notes come through; create one by hand on the QA Reviews page — see Send changes to a clinician.
