Which review tasks Clinical Managers own and where handoffs go next.

Clinical Managers own clinical review tasks for the patients on their team. They review orders, plans of care, OASIS items, and clinical blockers before the work moves downstream.

The Clinical Manager owns the team of patients, not only their own discipline. If a therapy order needs a substantial correction, collaborate with the therapist instead of handing the task off only because it is therapy-related.

Orders, POCs, and add-on evals

Open these tasks from Tasks. The patient chart shows the record, but the review task is the working surface for approval.

When a Clinical Manager completes a review order, plan of care, or add-on eval task, their signature is added to the resulting order as the clinical manager cosign. For a verbal order, the signature line may show the provider order trail, the provider's agent, the field clinician, and the Clinical Manager cosign.

After approval, Medical Records owns sending, signature follow-up, and filing the signed document back to the chart.

OASIS review

Use the Review and Lock OASIS task to work the review. The OASIS dashboard is for tracking locked and unlocked assessments and for export or iQIES follow-up after the review is complete.

Do not lock an OASIS while clinician approvals are still outstanding. The lock gate is zero pending clinician approvals, no denied items needing action, and cleared validation issues.

If something goes wrong

  • The task is assigned to the wrong person. Set the assignee to Unassigned or select the Clinical Manager or agency-approved reviewer.

  • The order is visible on the patient chart but cannot be approved there. Open the review task from Tasks and approve from the task screen.

  • A clinical blocker is unclear. Escalate to the Clinical Director, DON, or ADON with the patient, task type, blocker, and deadline.
