An add-on evaluation brings a new discipline onto an active episode — a PT or OT evaluation ordered mid-care. The clinician documents the eval; the Review Add-on Eval task is where you approve it and fold it into the plan.
Work the review
Open the Review Add-on Eval task from Tasks. The workspace has four tabs: Referral Order, Visit Notes, Order Details, and Attachments.
Read each tab and click Mark [tab] as Reviewed as you clear it.
Order Details is the editable tab — it carries the eval's plan content. Visit Notes is read-only context: narrative or routine documentation changes require an addendum from the clinician, not an edit here.
Complete and merge
Click Complete Review.
Your Clinical Manager signature is added to the order as a cosign.
The Merge with Plan of Care? dialog asks whether to fold the add-on evaluation into the patient's existing plan of care. Choose Yes, Merge when the eval's interventions and goals belong on the 485; choose No to keep it as a standalone order.
Note: If the patient's plan of care review is open at the same time, it shows a pending add-on banner — approve the add-on eval first so the plan review sees the complete picture.
Get the visits scheduled
Approving the eval does not plot the new discipline's visits by itself. The clinician's requested frequency arrives on its own provider order, where the visits are plotted on the calendar during review — see Approve a provider order.
If something goes wrong
Something is missing from the eval. Click Hold For Additional Review to park the task instead of completing with a gap, and follow up with the evaluating clinician.
You merged when you should not have. The plan of care shows the merged content — open the POC review, correct it there, and route the change as usual: see Review a plan of care.
