Late documentation and past-dated visits

Assign the visit on the actual date of care — never shift it to today.

The date of care must always be accurate. When documentation happens after the fact, assign the visit on the date care occurred — never shift the visit date to today.

Assign a visit in the past

  1. Open the patient calendar, or the assignment task.

  2. Plot or locate the visit on the actual date of care, even if that date has passed.

  3. Assign the clinician who performed the visit.

  4. The clinician completes documentation against that visit. The visit date, time in and out, and signature dates reflect the actual encounter.

The rules

  • Past-dated assignment is supported and expected for late documentation.

  • The notes table and the chart display the visit date, not the clinician's completion or signature date.

  • Do not create a same-day duplicate to work around a past date — that misstates the date of care.

Warning: A visit dated on the wrong day is a documentation integrity problem, not a scheduling quirk. The record must show when care actually happened.
