Correct visit documentation

Submitted notes are immutable; corrections use a narrative note or system addendum.

Once a visit note is signed and submitted, it is immutable — it is never edited directly. Corrections happen through one of two mechanisms, both of which preserve the audit trail.

General narrative note

Use General / Routine Narrative Note when the clinician documents a correction in their own words after submission.

  1. Open the patient → Notes.

  2. Click Narrative Notes.

  3. Click New Note.

  4. Select General / Routine Narrative Note.

  5. Document what was incorrect, what is correct, and why.

  6. Click Create Note.

Use this path for a frequency stated wrong, wrong laterality, or a missed detail.

PTA note cosign

Enzo does not support dual signatures on a submitted visit note. If the agency needs to document that a supervising PT cosigned PTA notes, the supervising PT enters a General / Routine Narrative Note.

Identify the PTA notes and dates being cosigned. State that the supervising PT reviewed and cosigns those notes.

Do not edit the submitted PTA note to add a second signature.

Addendum — generated by the system

Addendums attached to a visit note are created automatically when an OASIS change is accepted through the OASIS review flow. They record the original value, the new value, and the reasoning, and they print with the note. Clinicians do not write addendums directly.

Corrected frequencies flow through the order under review, and the provider signs the corrected order in the normal signature flow — see Approve a provider order.

If something goes wrong

  • A clinician edited nothing but the note looks different. Check for a system addendum — an accepted OASIS change appends one automatically.

  • The correction is urgent and the clinician is unreachable. The note stays as written until its author corrects it. Reviewers correct orders during review; they do not rewrite another clinician's note.
