Collaboration is internal communication; Notes is chart documentation.

Collaboration — internal agency communication

The Collaboration tab keeps the team's coordination beside the patient:

  • Tasks lists the patient's tasks, filterable by All, Open, In Progress, Overdue, and Completed — the same tasks as the main Tasks list, scoped to this patient.

  • Messages holds the Communication Log, filterable by All, Internal, Patient, and Provider. Conversations from the Communication app that reference this patient appear here — see Send and receive messages.

Everything on this tab is internal working communication. It coordinates the team. It is not chart documentation.

Warning: Do not document patient-care work in messages. Put that work in Notes as a visit note or narrative note.

Notes — chart documentation

The Notes tab holds the documentation for the episode:

  • Visit Notes lists each visit's documentation.

  • Narrative Notes holds patient-care documentation between and around visits.

Document work that contributes to the patient's care in Notes. Use Use narrative notes to choose the right note type.

If something goes wrong

  • Patient-care information ended up in a message. Document it in Notes → Narrative Notes. The message cannot be promoted into chart documentation.

  • A narrative note has an error. The note is corrected, never deleted — see Correct visit documentation.
