Upload and send documents

Patient documents, uploads, templates, and outbound tracking.

The Documents page manages what leaves the agency and what gets added by hand — the other half of the fax inbox.

Upload a document

  1. Open Documents and click Upload Document.

  2. Pick the file, set the document type — referral, order, clinical note, insurance, admin, discharge — and the patient it belongs to.

  3. The document files to the chart and appears under Patient Documents.

Use upload for paperwork that arrives outside the fax line: hand-delivered records, emailed PDFs, scanned consents.

Send documents out

Outbound documents — signed orders going back to physicians, records requests being fulfilled — appear on the Outbound tab with a delivery status:

Which channel a document goes out on — eSign, fax, email, or print — is decided by the provider's preferred method, not per document. See Transmission options for the channels and how signed pages find their way back.

Note: A medical records request from an outside party arrives as a task — see Work your tasks. Fulfill it by gathering the chart documents and sending them from here, then complete the task.

Attach documents to the chart

Documents can be attached to a patient chart from three places:

  • From the chart: open the patient's Documents tab and click Upload Attachment. The tab filters by episode (This episode / All episodes) and by type — F2F Encounter, Hospital Discharge Summary, Lab Results, Insurance Card, Plan of Care, Wound Photo.

  • From the inbox: use Link Document on a received document to file it to a patient, referral, or physician order — see Link documents to patients.

  • From an order: a returned signed document links to its order via Link Document → File to Patient Chart.

On the chart's Documents tab, use Designate as F2F on the qualifying encounter note — billing needs exactly one designated face-to-face document.

If something goes wrong

  • An outbound document shows Failed. Verify the destination fax number or address in the provider library, then resend.

  • An upload is on the wrong chart. Open it under Patient Documents and refile it to the correct patient; confirm it is gone from the wrong chart.
