Find your way around the patient chart

The chart's tabs and where to find physician, payer, meds, and visits.

The patient chart is the source of truth for patient care. Open a patient from Patients and choose the episode.

The header

The header stays visible everywhere in the chart: the patient's name, Signing Provider, and badges for MRN, DOB, SOC date, the Episode date range, and the primary insurance. The most-asked-for facts never require a tab.

The tabs

Use the fixed header for episode context, then move across the tabs for clinical, financial, and record details.

Start on Overview

Open Overview first for quick context. It shows who the patient is, the handoff summary, the episode calendar, and progress to discharge.

Use Overview to orient yourself. Use Notes to view visit notes for the episode, view narrative notes, or add a narrative note. Use Orders to view provider orders or add a new order.

Note: Messages about the patient sent through the Communication app also appear under Collaboration → Messages, so the chart carries the conversation history — see Send and receive messages.

If something goes wrong

  • A visit is missing from the calendar. Check the episode — the calendar shows one episode at a time, and the visit may belong to the previous one.

  • Information looks stale. Chart data follows the episode you opened. Confirm you are in the current episode before assuming something is wrong.
