Answers for field clinicians using the phone.

The app

Which tabs should I see? Schedule, Patients, Uploads, and Settings — plus Inbox when your agency uses team chat. If Schedule is missing, Scribe is not enabled for your organization; ask an administrator.

Why is my day empty? In order: confirm the organization in the header, tap the correct day on the week strip, check your discipline filter in Settings → Schedule View, then refresh. The phone reads a local copy — a visit assigned on the web moments ago can take a minute to appear.

Why does my schedule look different from a coworker's? Different organization or different discipline filter. Check the header org switcher, then Settings → Schedule View — RN/SN, PT, OT, and other filters show different visit types.

What is the difference between Schedule and Patients? Schedule shows your visits for a selected day. Patients lets you search any patient and open their profile, calendar, and visit list regardless of today's schedule.

Recording and consent

Why is Start Recording disabled? Tap Add Signature first. The patient — or their authorized representative, per agency policy — must sign before recording begins.

Can I use Scribe without recording? Yes. On Patient Consent for Recording, tap Continue without Recording. You document every field by hand. Follow your agency's policy for when recording is required versus optional.

What is the recording bar at the bottom? A red timer and Transcript control while audio is active. You can open documentation sections while recording continues.

Documentation

What is the Outline percentage? A checklist of required documentation for the visit type. Tap the badge to see what remains; it highlights red or amber until critical sections are done.

Do I re-enter documentation every visit, or can I pull it forward? Scribe fills in most of the visit from the recording, and the previous response to an item is surfaced in the visit so you can see what was answered last time. Nothing is copied forward automatically — the historical answer is there to compare against, not to reuse blindly.

What are unverified medications? Medications pulled from the referral packet that you have not yet confirmed the patient is actually taking. Verify each one during medication review — the point is to reconcile the paper record against what is really in the home.

Do goals and interventions come from a library or free text? Both. Select from the predefined intervention library (each with associated goals), or enter a custom intervention. Every aspect of a predefined goal is editable.

Where do suggested goals come from? The last post-visit question asks which body systems to include for goals and interventions. Answer it and suggestions build from your selections; skip it and they auto-populate from the referral and the visit transcript. Either way, review and edit before signing.

Does the medication list carry across disciplines? Yes. The list the RN builds at start of care persists; the next clinician reviews and reconciles it. Medication changes go through a provider order.

Are drug interactions checked? Yes — checks run against the FDB database during medication reconciliation. When an alert appears, record your clinical decision in the interaction note; the visit will not submit until every note is saved. Severe interactions require provider notification within 24 hours.

Submitting

Why can't I submit? The usual blockers: medications still marked need review, an active wound on file you did not address, required Outline items missing, or an upload still in progress. Review Your Visit names what is in the way — tap the row to jump back into the section.

Does Submit mean the visit is billed? No. Submit Visit As Is sends the encounter from your phone. Verification, coding, and billing continue on the web.

What does Sync Failed mean? You created the visit on the phone and the system found no matching scheduled visit. Contact your scheduler to add it on the web, or use Link Visit if documentation and schedule were created separately.

Offline and uploads

Does anything require signal during the visit? Documenting and submitting work offline. Reschedule and reassign, FDB medication search, and some order flows need a connection.

Can I reschedule or reassign offline? No — those requests go to the office live. If the submit fails, check Wi-Fi or cellular data and retry.

A recording shows failed. What now? Open the visit in Uploads and tap Re-upload on Wi-Fi — or Re-upload all failed. Stay connected until they finish.

Why does logging out warn me? Signing out removes the phone's encrypted local database, including recordings that have not finished uploading — and they cannot be recovered. Confirm Uploads is clear first.

Scheduling rules

How does the 14-day aide supervisory rule work? The system flags when a supervisory visit is not planned within every 14 days, or when one is moved out of its window — the same style of guardrail as the Medicare week.

What happens after I submit a visit frequency? The code you type (for example 2WK2, 1WK4) is validated as you type, then goes to the office as a confirm task. Approval plots the visits. One frequency submission per visit; later changes go through the office.
