Enter a provider order

Start an order from web or mobile, complete details, and submit it for review.

Provider orders can be entered from the patient chart, from the mobile patient screen, or from inside a visit. Each entry point uses the same order workflow.

The order is not approved and is not sent to the provider when it is first entered. Submitting the order creates a Clinical Manager review task.

Where to start

Use the entry point that matches where you are working:

Starting point

Path

Web patient chart

Open Patients, select the patient, open Orders, then select Add New Order.

Mobile patient screen

Open Patients, select the patient, open Orders, then tap the plus sign.

Inside a mobile visit

Open the visit, go to Orders, then tap the plus sign.

All paths open the same order entry flow.

Order details

Complete the order details first.

Field

What to enter

Type

Select Written when the order comes from an existing written source. Select Verbal when the order was obtained by phone or conversation.

Written order upload

For written orders, upload the original source when available.

Communicated With

For verbal orders, enter the provider or provider office contact who gave the order.

Order Entry Date / Time

Use the date and time the order is entered into Enzo.

Signing Provider

Select the provider who needs to sign this order. The list comes from the providers on the patient chart.

Order Obtained Date / Time

Use the date and time the clinician or agency obtained the order from the provider or provider office.

Secondary Provider

Select a provider who should receive an informational copy but does not need to sign.

Entered By

This fills from the user entering the order.

Order Description

Enter the full provider order text.

Use Secondary Provider when another provider should receive a copy only. For example, if the signing provider for this order is not the provider following the home health episode, send the episode signing provider an informational copy.

Content types

The Order Description states what the provider ordered. The content type tabs update the chart to match that order.

Select each content type the order changes:

Content type

Use it for

Medications

Add, edit, hold, or discontinue medications.

Diagnoses

Add, edit, remove, or re-sequence diagnoses.

Calendar

Add, remove, or change visits on the patient calendar.

Vital Sign Parameters

Add or update provider notification thresholds.

Procedures

Add or update wounds and procedures.

Interventions & Goals

Add, edit, or remove care plan interventions and goals.

Select only the sections that need chart updates. If the description says a visit, medication, wound, diagnosis, vital sign parameter, intervention, or goal changed, the matching content type must be selected.

See Update chart sections in a provider order for the content-type steps.

Review and submit

After the selected content type tabs are complete, the order opens Review & Complete.

  1. Review the order details.

  2. Review each content update.

  3. On web, select Preview Order PDF when you need to review the generated PDF before submitting.

  4. Use Back to return to a previous tab if something needs correction.

  5. Select Confirm Order or Submit when the order is ready for Clinical Manager review.

The order description is required before the order can be submitted.

What happens next

Submitting the order creates a Review Provider Order task for the Clinical Manager.

The order exists after submission, but it is still pending review. It has not been approved and has not been sent to the provider.

In the review task, the Clinical Manager may select Review / Edit Order, correct any order details or content type tabs, and save changes. The Clinical Manager must then select Approve Order from the task review screen.

After Approve Order, the order locks. If the order is set to send to the provider, Enzo routes it by the provider's configured delivery method. Manual-send orders move to the orders dashboard for Medical Records to send.

If something goes wrong

  • The signing provider is missing. Confirm the provider is on the patient chart. If the provider record is missing or incorrect, update the provider library before submitting the order.

  • A provider only needs an informational copy. Add that provider as Secondary Provider. The secondary provider is not asked to sign.

  • A visit does not appear after the order is entered. Confirm the order included the Calendar content type and that the Clinical Manager approved the order. See I entered an order. Why can't I see the visit?.

  • The order needs urgent approval. Do not rely only on the task queue for same-day care. Send a separate communication to the appropriate Clinical Manager or agency contact.

  • The order is wrong before approval. Open the review task, select Review / Edit Order, make the correction, and save changes before Approve Order.

  • The order is wrong after approval. Do not edit the approved order. Use a clarification order or dummy order. See Correct an approved order.
