Document each active wound: location, measurements, tissue type, drainage, and photos when policy requires. Mark no wounds if appropriate for the visit type.
Why am I blocked on submit with “active wound not documented”?
The patient has an active wound on file that you did not address this visit. Return to Wounds to document or acknowledge. On Review Your Visit, you may need to confirm twice if you intentionally skip an active wound.
Troubleshooting: Blurry wound photos — retake with adequate light and a reference ruler if required.
