The Tracking Logs tab in Quality Management holds five logs: PAEs (potentially avoidable events), Incidents, Infections, Complaints, and Surveys. Every entry follows the same shape — details, narrative, findings, communications, resolution, prevention, attachments — so log the event once and the record carries through investigation to closure.
Log an event
1. Open Quality Management → Tracking Logs, then the subtab for the event type.
2. Click the + button next to the subtab.
3. Fill in Details. Each type asks for what matters to it:
Log | Key details |
PAE | Event type, event date, initial severity, primary diagnosis, hospital or ED facility, length of stay, linked incident |
Incident | Incident type and subtype, date and time, reported by, location, severity — or mark it staff-only, property, or vehicular with no patient involved |
Infection | Who reported and to whom, date of onset, infection type and site, communicable or not, whether it was present on admission |
Complaint | Who complained and their relationship to the patient, when and through which channel, the nature of the complaint, whether it is a formal grievance or an immediate safety concern |
Survey | Survey type and method, date, overall rating, and ratings by domain |
4. Work down the remaining sections — Narrative, Findings, Communications, Resolution, Prevention, Attachments — as far as you know them today. You can return to fill in the rest.
5. In Prevention, set Add to QAPI tracking? to Yes when the quality committee should review the event.
6. Click the save button for the type — Save PAE, Save incident, and so on.
Track events to closure
Each log's table shows the event with its severity and status. Statuses move from New → Under investigation → Pending closure → Closed. Open an entry to update findings, resolution, and status as the investigation progresses.
Note: Severities are Low, Moderate, and High. High-severity events feed the Incidents tile on the Quality overview — log promptly so the overview reflects reality.
If something goes wrong
You closed the form and lost your work. Leaving an unsaved form asks Close without saving? — answer No to stay. Entries only persist after you click the save button.
An event is not in the log. Each subtab filters to its own type; check that you are on the right one, then clear the filters.
