Patient journey timeline

A cross-role timeline from referral through visits, orders, signatures, billing, and recertification.

Use this timeline to see how a patient moves from referral to active care, order review, signature follow-up, billing, recertification, and discharge.

For each task, the timeline shows who owns it, when it appears, what the owner does, and what happens next. Field Clinician represents the discipline-specific field roles in this timeline.

Key points along the journey

  • F2F review and F2F obtain tasks do not block referral acceptance. At SOC, the clinician drafts the plan of care and OASIS with the visit.

  • A held POC releases the first 7 days of visits while collaboration and review continue. POC approval releases the entire episode of visits.

  • POC approval generates Review and Lock OASIS. OASIS recommendations show the original answer, recommended answer, and reason for change in the clinician app.

  • NOA work starts from the completed SOC visit. Discharge uses Discipline Discharge for add-on disciplines and Agency Discharge for the main discipline.

  • Hospitalization starts with a transfer order. Resumption starts with a resumption order after the agency is notified that the patient discharged from the hospital.
