Operational reports
Day-to-day agency operations: referrals in, patients on service, visits happening, work getting done.

Admissions | Admissions within a date range, with drill-down by branch, team, payor, and coordinator |
Admissions (Non-duplicated) | Each patient counted once — separates new admits from readmits |
Current Census | The census snapshot right now, by status, branch, payor, and team |
Decline Reasons | Why referrals are being declined, broken down by category and payor |
Discharges | Patients discharged in a date range, with discharge level and discipline breakdown |
Do Not Readmit | Patients marked Do Not Readmit, filterable by branch, team, and patient |
Emergency Preparedness Report | Acuity status of active patients for disaster planning — by branch, county, evacuation zone, and case manager |
High Risk Patients | Patients flagged by M1033 risk criteria who have low visit frequency and no follow-up telephone visits |
Hospitalized Patients | Who is in the hospital right now, with transfer OASIS dates and facility details |
Insufficient Authorizations | Patients whose authorizations will not cover planned care, for Fee for Service and Case Rate payors |
LUPA and Outlier Analysis | Episode-level LUPA and outlier status — projected versus actual visits and variance to budget |
Missed Visits | Missed visits with rescheduling and provider-notification tracking |
Non-Admits | Patients who became non-admits, with referral details, visit status, payor, and coordination notes |
Non-Billable Visits | Non-billable visits for a timeframe, by branch, payor, and job code |
Notes | Clinical and operational note volume by type and author |
Orders | The physician orders pipeline — status and turnaround metrics |
Potentially Avoidable Events | CMS OASIS PAE data for public reporting — eligible episodes, event rates, and benchmark comparison |
Recerts Due | Active patients approaching end of episode, with visits to date, for recert planning |
Referrals | Referral activity and conversion by source, marketer, and outcome |
Tasks | Task workload and completion rates by type and assignee |
Time in Status | How long referrals sit in each workflow status before progressing |
Time to Accept | How long referrals take from receipt to acceptance |
Visits | Visit volume and completion rates by type, clinician, and status |
Financial reports
The revenue cycle: what has been billed, what is owed, what payers are doing, and how collections are performing.

"How are we growing?" — Admissions, Referrals, Time to Accept, Decline Reasons.

"Where is care breaking down?" — Missed Visits, Orders, Tasks, Potentially Avoidable Events.
"Where is the money?" — AR Aging, Billing, Denials, Payer Scorecard.
