The facility library

Add and maintain the facilities that referrals and team coverage use.

The facility library holds the organizations patients come from and return to — assisted living, skilled nursing, hospice, and the rest. Intake uses it for referral sources, and teams use it for territory coverage. It lives at Settings → Agency Management → Facilities.

Add a facility

  1. Click Add Facility. A form opens with two sections.

  2. Under Facility details, enter the Facility name, pick the Type — ALF, SNF, IL, Memory Care, or Hospice — and add the Primary contact and Phone.

  3. Under Operations, enter the Address and City, then the Total capacity and Current census. Capacity and census drive the utilization display on the list.

  4. Click Create facility.

To load many facilities at once, use Import.

Maintain the list

The directory shows each facility with its Type, Location, Primary Contact, and Utilization. Use the search box to find one, then edit from its row — update the contact when the facility's staffing changes, and keep census current if you track utilization. Delete facility removes an entry entirely; prefer editing over deleting anything with referral history.

Where facilities are used

  • Intake — referrals name a facility as the source; a clean library means intake picks from a list instead of retyping.

  • Team coverage — teams can claim facilities as territory, which routes that facility's referrals and visits to the right team (Settings → Teams).

If something goes wrong

  • A referral's facility is not in the list. Add it here first, then finish the referral — do not free-type a name that only exists on one referral.

  • Utilization looks wrong. It is computed from Total capacity and Current census on the facility — update those fields.

  • Two entries for the same building. Standardize on one and update team coverage to point at it before removing the other.
